Queen of Hearts was a Gotham City serial killer and enemy of Batman.

She was a perfectly normal woman, until she became convinced to be suffering from a lethal heart condition, and believed herself to be in need of a transplant. Wanting to help the doctors, she armed herself with a drill, and she stormed the city's streets targeting young drug dealers: she killed them with the drill, opened their rib cage, took their hearts and then she put them in tiny, heart-sized baby cribs, ready for the transplant she believed she needed. Soon enough, the thrill of the hunt got the best of her, and she kept murdering dealers: following Gotham's fashion for serial killers, she tailored herself a costume, and renamed herself the Queen of Hearts, quite appropriate considering her M.O., after all. Murdering people in Gotham inevitably attracted the attention of it protector, and the Queen didn't fail to do so: finally found by Batman, she confronted him on a rooftop, and during the fight, she slipped over the edge, falling down. Batman caught her in midair, and in a single moment of lucidity the Queen begged him to let her fall to her death, since she couldn't fight her urges and she was sure she would have killed again if given the opportunity. Batman saved her nevertheless, and sent her to Arkham Asylum, where he hoped she could be cured. It turned out the one who was right was the woman, eventually.

Three years later, the urge to kill took the best of the Queen of Hearts again, and she escaped from Arkham. She decided to change hunting ground, and she moved to Rio de Janeiro, Brazil, where she resumed her old habits. With such a unique M.O., it was only a matter of time before Batman traveled to Brasil to see if it was truly the Queen of Hearts the one behind all those murders. Oblivious to the fact that Batman was in the city, the Queen had already selected her new target, a young drug dealer named Zeno: she chased him to the apartment of his supplier, a scientist named Crosby, and just as she was about to kill them both, she was interrupted by Batman. As the two were busy fighting, they made the mistake of breaking several vials in Crosby's lab, inhaling the fumes coming from them: the two enemies were then transported to a psychic dimension, the Idiot Zone, inhabited and ruled by a cruel deity, the Idiot, the one responsible of the dozens of zombie-like kids roaming the streets of Rio in a catatonic state. There was a new drug in Rio, produced by Crosby and sold by Zeno to kids, a drug that could transport human minds to the Idiot Zone, where the Idiot could feed on them. The Idiot appeared before the Queen and Batman, and presented himself as the ruler of that dimension: he asserted he could drain a mind blank by overloading it with nonsensical thoughts, and he decided to give a proper demonstration of his powers using the Queen of Hearts as a guinea pig. Before she could even realize what was happening, the Idiot grabbed the Queen's head, and he filled her psyche with so many senseless images and paradoxes that it shut down in a matter of seconds, reducing her to a withered husk, in a permanent catatonic state. The Queen wasn't dead, but in this state she would have most certainly not been able to kill anyone ever again.
